Form Filling Guidelines

• Form type-Form 49 A
• Any one of the self help group executives should sign inside  the signature box
• No need to place seal in application form(refer sample form)
• The Name of the women self help group Should be filled in Last Name of the Application Form
• If the Name is More than 26 Character length, then you can fill the remaining characters in First Name
• Card Name Should be filled in 2nd Column
• In the Fifth Column ,The self help group registration date, registration month and registration Year Need to be filled
• The Women Self Help group Address Need to be filled in 7th column Residence Address
• Need to select Residence in 8th column
• Fill Country code, Mail id and phone number in 9th Column.
• Need to write the Name of the Person in 16th column who is signing in the form
• In  16th column in the capacity field, if the signee is male need to fill “HIMSELF”. if the signee is female, “HERSELF” need to be filled
