MINOR PAN CARD

Documents :-

• Two passport size photo of the minor(Applicant)
• Aadhar copy of the minor(Applicant)
• Aadhar copy of the Parent(Father or Mother)/ Guardian

Form Filling guidelines :-

• Minor’s photo should be affixed in the form
• Signatuer should be done by parent
• Parents details should be filled in the 14 th column of the application form
• Minor name should be filled in the 16 th column (I/WE)
• In the 16 th column replace “In the capacity ” with “RA” instead of Himself or Herself
